Vote-By-Mail Requests require name, date of birth, address in Montgomery County, address where you want your Vote-By-Mail ballot sent, and handwritten signature. Signature is critical on applications, and we will accept a scanned document submission. Voter's original signature is required to be affixed on the returned ballot envelope under the oath.
